To equalize the monthly cost of utilities for customers of the City of Hillsboro. The amount of the level payment will be based on actual or estimated usage during a twelve month period at customers current address. Customer must be current with their present billing in order to qualify for the level payment plan.
The customer shall be entitled to receive City utility service under the level payment plan provided the customer agrees to the following:
A. To pay each level monthly installment on or before the due date thereof.
B. That failure to pay any level monthly installment, on or before its due date, shall be cause for termination by the City of Hillsboro of the level payment plan with respect to customer, in addition to other remedies permitted by the City’s rules and regulations.
C. That the estimate shall apply only to the premises then occupied by customer, and that if such premises are vacated during the period covered by said estimate, the level payment plan, with respect to customer, shall immediately terminate.
D. That if the plan is terminated, any amount or amounts payable by or due to customer on account of the metered service during the period covered by the Plan, shall be billed or credited to the customer at once.
E. That until terminated by either party, the plan will be renewed automatically.
F. That the level payment plan accounts may be periodically reviewed by the City, and the level monthly installments payment provision shall be revised, if it appears at the time of review, that the debit or credit balance at the end of the twelve month period will substantially exceed the estimate.
G. That the difference between the accumulated total amount of the customer’s billing determined by meter usage, and the accumulated total of the amounts paid before the final month of the annual period, shall be charged or credited, as the case may be, to the service bill for the final month of such annual period, which shall be subject to current settlement before the start of the next annual period.

I agree to pay the sum of $_______________ per month for utility service at my residence as shown above for a period of eleven (11) months beginning ___________________________ with an actual amount payable on the twelfth (12) month as shown on the billing card, if any is shown. Payments are to be made on or before the 15th of each month without fail.
I acknowledge that I have read, and that I understand City Policy No. 43 which lists the conditions that allow me to participate in the City’s level payment plan.
I also understand that failure to abide by the terms of this agreement will render this agreement null and void, and my utilities, under those circumstances, could be disconnected.
